Lens-Induced Glaucoma

Lens-induced glaucoma is a clinical disorder that occurs with the delay of cataract extraction and is more common in developing countries. It can be seen as open angle (Phacolytic glaucoma, phacoantigenic or phacoanaphylactic glaucoma, lens particle glaucoma) or closed angle (phacomorphic glaucoma, lens dislocation) glaucoma. Phacomorphic, phacolytic and phacoantigenic glaucomas are respectively associated with narrow anterior chamber, corneal edema and increased intraocular pressure (IOP) in the presence of intumescent cataract; hypermature cataract with intact capsule, lens proteins and flare in the anterior chamber; and ruptured capsule, increased IOP with flare in the anterior chamber. The intraocular pressure is usually above 40 mmHg. Initial urgent medical treatment should be started, but defi nitive treatment is lens extraction. Patients and surgeons should be educated in terms of early diagnosis and treatment because of unfavorable visual outcomes.
