In the ophthalmic examination, there was right upper eyelid ptosis, enophthalmus, upper displacement of the right eyebrow and the right scin crease. According to the detailed medical history of the patient travaprost eyedrops was being used for 1.5 years. In this case report we emphasize that travaprost, which is a topical prostaglandin analog, has an antiadipogenic effect leading to prostaglandin related periorbitopathy. We also emphasize that unnecessary cosmetic surgeries must be avoided since this effect is reversible when the eyedrop usage is discontinued. Ophthalmologists should be aware of this potential side effect of topical travaprost, especially with monocular use as it ends up with facial asymmetry.
