Visual Field Changes in Glaucoma: 4 Years Perimetry Follow-up

Purpose: To determine the possible changes in visual field in patients with primary open angle glaucoma (POAG) in 4 years follow up.

Materials and Methods: 100 eyes of 50 patients (23 female, 27 male), who had POAG diagnosis and medically controlled intraocular eye pressure, were included in this retrospective study. Intraocular pressure was measured with Goldmann applanation tonometer and visual field was measured with Octopus 900. Measurements were repeated every 6 months along 4 years follow up time. Possible changes in visual field research with EyeSuite™ Software Analysis program.

Results: Mean age was 52.24±13.76. At the beginning of study mean intraocular eye pressure was 17.65±2.26 mmHg (range: 11-20). All patients had normal intraocular eye pressure (lower 21 mmHg) in every 6 months visits. Visual field loss in favor of glaucoma was detected in 20 eyes (20%). There was not significant intraocular eye pressure difference between the eyes which have new visual field loss and the eyes which have no changes (p=0.279).

Conclusion: A new visual field defects are detected at %20 of eyes who were followed for 4 years despite of medical therapy which thought to be effective. The cases which glaucomatous damage had increased although patient's normotansive state shows us there were other factors affecting visual field defect. We think that custom target intraocular pressure value has to be decided for every patient, also use of neuroprotective medicines or optic disc blood flow may be effective factors.
