Long-Term Outcomes of Phacoemulsification and IOL Implantation Without Capsuler Tension Ring in Traumatic Cataract with Zonular Damage

Süleyman Demirel Üni. Tıp Fak. Göz Hastalıkları A.D., Isparta Purpose: To evaluate the long-term results of phacoemulsification and posterior chamber intra ocular lens (IOL) implantation without capsuler tension ring (CTR) in traumatic cataract with zonuler damage.
Materials and Methods: Fifteen eyes of 15 cases (10 male, 5 female) having traumatic cataract and zonuler damage up to one quadrant were evaluated in this retrospective study. Phacoemulsification and posterior chamber IOL without CTR was performed in all cases. Eleven cases (73%) had non perforating eye trauma and 4 cases (27%) had perforating eye injury history. Intraoperative IOL stabilization were enough and CTR was not used. In all cases, hydrophilic acrylic IOL with PMMA haptic were implanted. In 6 eyes (40%) phacoemulsification and pars plana vitrectomy were performed in the same session.
Results: The mean age was 46 years (range 27 to 59). Preoperatively, 2 cases (13%) had intraocular foreign body, 3 cases (20%) had iridodyalisis, 1 case (7%) had retinal detachment, 5 cases (33%) had vitreous hemorrhage, 5 cases (33%) had elevated intraocular pressure, 1 case (7%) had glaucoma history. The median time between trauma and surgery was 4.8 months (range 1 to 240 months). The mean follow up time was 15.7 months (range 8-22 months). After surgery, in all cases except one IOL was stable and intraocular pressure was within normal limits. Visual acuity improved in 12 cases whereas did not improve in 3 cases due to the posterior segment pathology.
Conclusion: In the long-term, eyes having traumatic cataract with zonuler damage up to one quadrant posterior chamber hydrophilic acrylic IOL with PMMA haptic implantation without CTR was found as anatomically and functionally applicable.
