The Long Term Results of Primary Trabekulectomy

Purpose: To evaluate early and long term results in primary trabeculectomies performed for different types of high tension glaucomas.

Materials and Methods: Medical records of the 319 eyes of 277 patients undergone trabeculectomy operation with the diagnosis of primary open angle glaucoma, angle closure glaucoma, capsular glaucoma and secondary glaucoma in Ankara Numune Training and Research Hospital 3 st Eye Clinic between December 1993-January 2003 were evaluated retrospectively. The cases having an intraocular pressure (IOP) of 21 mmHg or lower with or without medication were accepted as successful.

Results: One hundred and seventy seven patients (63.9%) were male and 100 patients (36.1%) were female. Their mean age was 61.2±12.3 years and mean follow-up period was 45.5±29.5 months. Complete success rates (without medication) of the operations were calculated as 64.6% in the 1st year, 61.2% in the 5th year and 58.8% in the 8th year. Success rates with antiglaucomatous medication were 92.2%, 85.2%, 84.5% in the 1st, 5th and 8th years respectively. The decrease in IOP in respect to the preoperative values was statistically significant and there was no difference between postoperative values (p>0.05). The decrease in antiglaucomatous medication number was statistically significant (p<0.001). Mean c/d rates were 0.74±0.27 preoperatively and 0.78±0.26 postoperatively. The success of the trabeculectomy was not effected from the preoperative medications, the technique of conjunctival incision and the shape of scleral flap (p>0.05).

Conclusion: Primary trabeculectomy was found to be successfull in the last postoperative period as in the early postoperative period in cases when the antiglaucamatous medication was insufficient or unsuitable. Keywords : Trabeculectomy, glaucoma, intraocular pressure
