One Brother and Two Sisters with Developmental Glaucoma: Particular Conditions at the Treatment

In our study we evaluated the clinical picture and treatment approaches of a male patient whose one eye is buphthalmic (the other eye is phthisic) and his two sisters who came to the clinic with insistent questioning. We performed trabeculectomy combined with MMC to the male patient’s only eye which was buphthalmic, highly myopic and had uncontrolled glaucoma although the maximum medical treatment. Then we made needling because of the tenon cyst development. At the end of these procedures the intraocular pressure (IOP) was under control and we managed to protect the visual acuity that the patient had before the operations. For one of the sisters whose one eye is buphthalmic and the other eye has late-onset developmental glaucoma we performed trabeculectomy with MMC for the buphthalmic eye that has visual acuity of light perception, the other eye’s IOP was brought under control with medical treatment. The second sister had keratoplasty for one eye many years ago at a different medical center and the other eye was perceiving only the hand movements and this eye had cataract and glaucoma. For this eye we made lens extraction+Ahmed valve implantation and we supported the very weak sclera with allograft (Tutoplast).
All the families with developmental glaucoma must be questioned and examined for both the individuals who are ill and for late-onset cases. In those difficult cases glaucoma operations are very special needing careful technique and follow-up.
