Long Term Results of Cyclodiode Photocoagulation for Refractory Glaucoma Following Penetrating Keratoplasty: Experience of 10 Years

Purpose: To investigate the efficacy of diode laser cyclophotocoagulation on refractory glaucoma after penetrating keratoplasty.
Materials and Methods: 18 eyes of 16 patients with refractory glaucoma following penetrating keratoplasty who underwent diode laser photocoagulation were evaluated retrospectively. Patients with less than 1 year follow-up were excluded. All patients received 18-20 applications for 2 seconds over 270° under local or general anesthesia. The intraocular pressure (IOP) differences, visual acuity, graft status and complications were recorded.
Results: Of 16 patients, 8 were female and 8 were male. The mean age was 40.89±19.75 (9-90 years). The mean follow-up period was 46.72±28.81 months. At the final examination IOPs between 6 and 21 mmHg were achieved in 16 (88.8%) eyes. Mean IOP decreased from 29.8 mmHg to 15.4 mmHg. More than one treatment was needed in 6 (33.3%) eyes. Phthisis bulbi did not develop in any patients.
Conclusions: Secondary glaucoma is a severe vision-threatening complication for patients with penetrating keratoplasty. Diode laser cyclophotocoagulation is a useful and safe treatment modality for refractory glaucoma following penetrating keratoplasty but multiple treatments were required in some cases.
