The Effect of Trabeculectomy on Corneal Endothelial Cells

Purpose: To evaluate the effect of trabeculectomy on corneal endothelial cells and related factors.
Materials and Methods: 39 eyes of 36 patients who had undergone trabeculectomy were evaluated prospectively. Age, intraocular surgery history, pseudoexfoliation, diabetes mellitus, glaucoma type, intraoperative mitomycin-C (MMC) and postoperative 5-Flourouracil (5-FU) use and postoperative complications were recorded. Preoperative, postoperative 1st, 3rd and 6th month best corrected visual acuity, intraocular pressure (IOP), endothelial cell area and count per mm2 measured by specular microscope were compared.
Results: Endothelial cell count was 2167.2±551.1 preoperatively, 2119.5±618.2 in the postoperative 1st month, 1908.8±661.8 in the postoperative 3rd month and 2163.7±658.4 in the postoperative 6th month. There were no statistical significant differences between pre and postoperative endothelial cell areas or counts (p>0.05). A statistically significant IOP drop was obtained in all postoperative controls compared to preoperative IOP (p<0.05). We observed that eyes which had a history of intraocular surgery and narrow angle glaucoma had lower endothelial cell counts preoperatively. We observed that intraoperative MMC and postoperative 5-FU had no effect on endothelial cells but postoperative anterior chamber narrowing decreased the endothelial cell count.
Conclusion: We suggest that eyes with or intraocular surgery history, narrow angle glaucoma and postoperative anterior chamber narrowing should be evaluated more carefully with endothelial cell count before and after trabeculectomy.
