A case, which presents with acute bilateral angle-closure glaucoma and acute myopia, was seen at our clinic. Patient had no prior history of glaucoma and/or any other ocular abnormalities but had a history of secondary generalized epilepsy and was started with topiramate acetate 10 days prior to admission. Patient’s signs and symptoms responded well to topiramate discontinuation, topical and systemic anti glaucomatous medications and topical cycloplegic agents. In the follow-up period, visual acuity, intraocular pressure, gonioscopic and A-scan ultrasonographic findings returned to normal. Due to the potential ophthalmic side effects of topiramate, patients should be warned prior to drug initiation. Keywords : Topiramate acetate, epilepsy, angle closure glaucoma, myopia
