To present the clinical and imaging findings of a patient with a lesion behind iris. A 30-year old female patient admitted for her routine ophthalmic examination. She had urea cycle defect according to arginaz deficiency and mental motor retardation in her systemic assesment. Binocular best corrected visual acuity was counting fingers at 3 meters. In her slit lamp examination a posterior camara intraocular lens and a mass at the inferotemporal quadrant of the iris was noticed at the right eye. There was a posterior camara intraocular lens at the left eye. Fundoscopic examination was normal. Lesion was assesed via visante anterior segment optical coherence tomography and ultrasound biomicroscopy (UBM). In the light of these findings patient was diagnosed as iris pigment epithelial detachment and reactional fibrosis due to the optic part of the intraocular lens entrance between the iris pigment epithelial. Lesion size was followed with 6 months intervals and no change was found. Iris lesions composes important diseases in terms of differantial diagnose. Visante anterior segment optical coherence tomography and UBM were very useful devices for differantial diagnose of these lesions. Keywords : Iris pigmet epithelial, ultrasonic biomicroscopy, visante anterior segment optical coherence tomography
