²M.D. Professor, Marmara University Faculty of Medicine, Department of Ophthalmology, Istanbul/TURKEY Even though the exact mechanism is unclear, phacoemulsification leads to a mild but long lasting intraocular pressure reduction. It has been shown that cataract extraction positively contributes the treatment by reducing intraocular pressure in eyes with acute and cronic angle closure glaucoma. However, its effect on open angle glaucoma and ocular hypertension is still controversial. When this relation is unveiled, it would be possible to predict target intraocular pressure more or less and organize the entire treatment in such patients. In this literature review, intraocular pressure alterations and possible mechanisms of this change both in short- and long-term records following phacoemulsification have been revised.
