A Sixty two-year-old diabetic male patient applied to our clinic with a complaint of decreased vision in the right eye. On examination, visual acuity was 5/10 in the right eye and 8/10 in the left eye. Patient's medical records showed an uncomplicated phacoemulsification surgery and hydrophobic acrylic IOL implantation in both eyes in 2006. About a year ago, YAG lazer posterior capsulotomy was performed due to a posterior capsular opacification in the right eye The biomicroscopic examination with dilated pupil revealed microvacuoles over the IOL in the right eye. The IOL in the left eye was normal. Fundus examination of both eyes showed peripheral laser spots.

IOL microvacuolization that is particulary seen in patients to whom hydrophobic acrlylic IOL was implanted rarely results in a reduction in visual acuity .The laser applications, and/or medications which are used after laser application may be the causes of IOL microvacuolization. We recommend the use of nonglistening IOLs in patients with a disease which disrupts the blood-aqueous barrier or in circumstance in which chronic ocular drug use is inevitable. Keywords : Glistening, microvacuoles formatıon, hydrophobic acrlylic IOL
