Real Way: Canaloplasty

Although trabeculectomy is the gold standard for surgical treatment of glaucoma, numerous short- and long-term complications, including hypotony as an important visionthreatening complication and late blebitis, limit the use of this technique. Therefore, alternative glaucoma surgeries such as glaucoma drainage implants, deep sclerectomy and viscocanalostomy have been put into practice in the last decade. However, these newer techniques have complications too, and so surgeons have continued to investigate physiologic, anatomical, and less complicated bleb-free procedures. Increased technological developments prompted a renaissance in Schlemm's canal surgery recently.
Canaloplasty is a newer technique that makes functional collapsed glaucoma aqueous drainage system of Schlemm's canal again by 360 degree circumferential viscodilatation and tension of the inner wall of Schlemm's canal. In this article we discuss the indications, contraindications, complications, and technical specifications of canaloplasty surgery.
