Posterior Subcapsular Cataract Formation After İntravitreal Triamcinolone İnjection

Purpose: To evaluate the occurrence of posterior subcapsular cataract (PSC) after intravitreal triamcinolone acetonide (IVTA) injection and the factors associated with its development.
Materials and Methods: 173 eyes of 135 subjects in whom 4 mg IVTA was injected due to various etiologies were included. The association between PSC occurrence and age, intraocular pressure (IOP) increase, diabetes mellitus (DM) existence and the duration between PSC diagnosis and the IVTA injection was analyzed.
Results: 74 female and 61 male were studied. The mean age was 61.7±10.32 (22-83) years old. The indications for IVTA injections were; diabetic macular edema in 115 eyes, the concomitant therapy with photodynamic therapy (PDT) for age related macular degeneration (AMD) in 33 eyes, macular edema due to retinal vein occlusions in 19 eyes (branch or central retinal vein occlusion), uveitic macular edema in 5 eyes and macular edema secondary to macroaneurysm. The follow-up period was 15.4±8.5 (4-36) months. The PSC formation was more frequent in diabetic subjects (28.6%) compared to nondiabetics (13.7%) (p<0.05). In patients followed-up for at least 12 months, PSC was more frequently developed (30.4% versus 11.4%; p<0.01). In 24.2% of the cases steroid-induced IOP elevation was treated with antiglaucoma medication. Age, sex and IOP increase was not associated with PSC formation.
Conclusion: The occurrence of PSC increased after IVTA injection especially after 12 months follow-up period. Additionally DM was found to be a risk factor for the development of PSC.
