Capsular block syndrome (CBS) is a rare complication of cataract surgery. Small capsulorhexis diameter, inadequate removing of the viscoelastic substance at the end of surgery and remaining cortical remnants are the most important risk factors for the development of CBS. In this case report, four cases with CBS are presented with the following signs: slight anterior chamber shallowing, forward displacement of the intraocular lens (IOL), posterior capsule distension, closure of the anterior capsule aperture with IOL optic, and accumulation of transparent or milky fluid between IOL and posterior capsule. All cases were treated successfully with Nd:YAG laser anterior or posterior capsulotomy. Keywords : Capsular block syndrome, cataract surgery, laser capsulotomy
