Per-Op and Post-Op Complications of Cataract Surgery in Eyes with Pseudoexfoliation Syndrome

Purpose: To evaluate preoperative and postoperative complications and therapies in patients with pseudoexfoliation.
Materials and Methods: Fourtyone eyes of 28 patients with pseudoexfoliation operated with phacoemulsification procedure between March 2004 –March 2006 were included in this retrospective study in Dr.Lutfi Kirdar Kartal Training and Research Hospital 1.st Eye Clinic. Sixteen (57.2%) patients were women and 12 (42.8%) were men with a mean age of 68.37±3.26 (60-76). We analyzed intraocular pressure (IOP), presence of pseudoexfoliation, anterior segment and fundus appearance. Pseudoexfoliation was unilateral in 15 cases (53.5%) and bilateral in 13 (46.4%) cases. We evaluated preoperative and postoperative complications and therapies.
Results: Zonular dialysis was observed in 7 (17%) eyes peroperatively. In 2 (9.7%) eye we had to change phacoemulsification to extracapsular cataract extraction, because of the zonular enhancement. In 6 (14.6%) eyes posterior capsule perforation and vitreous loss occured peroperatively and we applied anterior vitrectomy. Foldabl hydrophilic acyrlic intraocular lens (IOL) was implanted in 33 (80.4%), sulcus polymethylmetacrylate lens was implanted in 6 (14.6) comlicated eyes and in 2 (4.9%) eyes IOL was not implanted. The measurement of intraocular pressure at 1 day was 24-35 mmHg (mean 25.7±4.2), in 5 (12.1%) complicated eyes and in 2 (4.8%) uncomplicated eyes. Intraocular lens dislocation was occured in 1 (2.4%) eye after 1 month.
Conclusion: Pseudoexfoliation is determined as a risk factor that increases the rate of complication of phacoemulsification. The risk of zonular dialysis, posterior capsule perforation vitreous loss peroperatively and IOL dislocation postoperatively are high. Solutions of the problems may effect the visual acuity.
