Allergic Conjunctivitis and Elevated Intraocular Pressure Associated with The Use of Brimonidine Tartrate 0.2%

We report allergic conjunctivitis and elevated intraocular pressure (IOP) associated with the use of brimonidine tartrate 0.2% in a 68-year-old female patient. The patient was using brimonidine tartrate 0.2% twice daily for 8 months. For the last 2 months, she complaint of severe redness, tearing, pain and blurred vision and had been treated with topical antibiotics and steroids with the diagnosis of conjunctivitis and episcleritis. On examination, the conjunctiva was extensively hyperemic and edematous in both eyes and there were multiple giant follicules. The IOP was 35 mmHg in the right eye and 45 mmHg in the left eye. When brimonidine was stopped, the IOP was controlled with the combination of topical beta-blocker and carbonic anhydrase inhibitor and the allergic reaction resolved in a month.
