Results of Trabeculectomy with Mitomycin C in Patients with High Risk of Surgical Failure

Purpose: To evaluate the results of trabeculectomy with mitomycin C in patients with high risk of surgical failure
Materials and Methods: Between April 1998 and December 2003, 47 eyes of 45 patients with uncontrolled glaucoma by maximally tolerated medical treatment were enrolled in this study. All eyes underwent trabeculectomy with mitomycin C 2 % concentration for 3 minutes. Kaplan-Meier survival curve was applied.
Results: There were 30 males and 15 females with a mean age of 64.5 (range, 7-83) years. All of the patients were followed-up with a mean period of 14 (range, 10-38) months. The mean preoperative intraocular pressure was 31.6 ( range 22-60) mmHg. There were 14 (29.78%) eyes with pseoudo-exfoliative glaucoma, 10 (21.27%) with glaucoma secondary to keratoplasty, 10 (21.27%) with primary open angle glaucoma, 5 (10.63%) with neovascular glaucoma, 4 with pseoudophakia (8.51%), 2 (4.25%) with aphakia, 2 (4.25%) with primary close angle glaucoma. Fourty-two eyes had previous intraocular surgery (cataract surgery in 17 eyes, keratoplasty in 10 eyes and trabeculectomy in 15 eyes). At last follow up 40 (87.23%) of 47 eyes had intraocular pressure under 21 mmHg.
Conclusion: Trabeculectomy with mitomycin C is an effective surgical procedure with low postoperative complication and high success rate in high risk patients.
